Back Pain? It’s Probably Not Your Herniated Discs

Your herniated disc probably isn't causing your pain. Learn the truth and do something about it!

Acording to many studies, up to 85% of people in the United States will have an episode with low back pain.  In fact, low back pain is second only to upper respiratory infections (the common cold) as a reason for missed work time!  That's right, more people miss work due to low back pain than any sickness under the sun besides the common cold, pretty staggering. So why do so many of us have low back pain?  The answer can be complicated, so instead, much of our medical community has come up with a scape goat who you probably have heard about. The vaunted "lumbar (low back) disc herniation" or "lumbar disc bulge".  So what is a disc herniation and how is it different from a disc bulge?

To understand a disc injury, first you have to understand a disc.  Very simply put, a disc is a shock absorber for your spine.  Instead relying on bones to buffer themselves from injury when we do high impact activities like walking, jumping, running, and lifting weights, the human body has an intricate system of shock absorbers (discs) between each spinal bone.  A disc is not much different than a jelly doughnut in regards to its anatomy and phsiology.  The outside of a disc (annulus) is a pliable but hard series of cartilage rings while the inside of the disc (nucleus) is a viscous fluid made mostly of water.  Think of the cartilage as the doughnut and the fluid as the jelly in your jelly doughnut.  In healthy discs, when we bend forward, the fluid goes backwards, and when we bend backwards, the fluid moves forward, and so on and so forth.

A herniated disc as seen in the picture attached to this article is what happens when the pressure on the nucleus (jelly) becomes so great that it actually pushes through the annulus (doughnut).  In most cases, perhaps as high as 90% of the time, this type of injury will happen in the back of the disc on either the left or right side (dangerously close to where the nerves come out of the spinal cord).  A disc bulge as seen in the picture attached to this article as well is what happens when pressure on the nucleus of the disc becomes great enough that it tries to force its way out of the annulus unsuccessfully.  Basically, it forces a weak spot to develop in the outside cartilage but never gets all the way through.  Both herniated and bulging discs can be seen clearly on MRI (Magnetic Resonance Imaging).  They cannot however be decisively diagnosed on an x-ray.  An x-ray, or plain film radiograph is used as a precursor to the MRI and can suggest the presence of a herniation or bulge without definitive confirmation.

So now you know what disc herniations and bulges are.  Maybe you have one.  Your medical doctor, orthopedist, chiropractor, or physical therapist has told you or someone you know, "you have herniated discs in your back and this is why you have back pain."  Okay, problem solved, you know what has caused your symptoms, and there isn't much you can do about it if you don't want surgery according to your doctor of choice.  Well, what if I told you that according to the most recent research, the presence of disc herniations and disc bulges in MRI are not at all conclusive as being causative for low back pain?  I promise you, I'm not crazy.  I brought some of my heavy hitters with me to prove my point.

In a ground breaking study performed by the New England Journal of Medicine, 98 asymptomatic (pain free) people were given MRI examinations to determine the presence or absence of lumbar sic herniations. The findings were surprising.  Of the 98 individuals examined only 36% (about 36 people) had what doctors consider "normal discs at all levels".  Over 52% had at least one disc bulge while 27% had at least one disc herniation!  The conclusion of the study... "The discovery by MRI of protrusions (disc herniations) in people with low back pain may frequently be coincidental."  I'm sorry, what?  That can't be right.

In a follow up study, the Cleveland Clinic performed the same clnica trials over and over again imaging hundreds of subjects.  Their findings... "25% of all asymptomatic people who climb into an MRI machine will have a herniated disc... As many as 60% of healthy adults with no back pain have degenerative changes in their spines".  What!  Seriously? Yes.  The Cleveland Clinic study was even published in the New York Times back in 2008!

Let's take this one step further because those results can't be right.  Dr. Modic of the Cleveland Clinic wanted to examine symptomatic patients.  250 patients to be exact, all of whom had back pain or pain shooting down their leg which can commonly be caused by herniated lumbar discs.  The findings were that 60% of the subjects had herniated discs.  "Ah ha" you say, the number goes up in people with pain.  Yea, it does.  But check this out.  Six weks after the initial scan, another MRI was performed to determine the relationship between the patient's symptom changes and the imaging findings.  The results, in 13% of the subjects the herniated disc had actually grown larger and in 15% the herniation had disappeared all together.  The interesting part is that there was no, zero correlation between the presence of the disc injury on the second scan and patient symptoms.  Some patients with no disc injury on the scan had gotten worse, and some with worse disc injuries on the scan had improved.  How do we explain this?!

It's really simple.  The disc is not likely the primary cause of the patient's discomfort.  It is simply a radiological finding that doctors should put in their notes while they keep looking for what your real problem is.  Don't get be wrong, there are instances where disc injuries can be so bad that surgery is needed to alleviate them, and in those cases surgery should be performed.  However my point is that disc injuries are typically unreliable as pain generators and should not be the scape goat for your low back pain.  Don't worry, I'm not going to leave you hanging, I'll tell you what the problem is.

Compensation.  Your body was meant to work a certain way.  Every muscle, every joint, every nerve, and every cell has adistinct, unique purpose.  When something breaks down, something else has to pick up the slack.  My favorite analogy for disc herniations is a crew team.  If there are five people in a boat rowing for a common cause, there is a definite speed that they can achieve for a definite distance.  If we take one of the crew members out of the boat (injure a disc or other tissue), the other four team members have to work 25% harder each to replace the fifth team member.  Eventually, their pace slows, the form breaks, and the boat stops being effective.  This happens in your body as well.

If you injure a disc in your low back, which as you now know is extremely common, your body becomes littered with compensations.  Other muscles and, structures begin picking up the slack for the guy who abandoned ship only to break down eventually.  And what happens when your body breaks down?  Pain.

Fortunately your body is extremely efficient at replacing losses by compensating for them.  Unfortunately, this can be a painful, and potentially debilitating.  You might be suffering from it right now, you definitely know someone who is.  So you have two choices.  Deal with it until the pain (not the compensation, but the pain) goes away, or try and get it fixed.  Either way, now you know, and knowing as they say is half the battle.

Until next time...  Have a good day, we'll talk again soon

TRIVIA: Joseph Carlino, former Speaker of the NY State Assembly, gave a speech on the 5thRead More Amendment at my high school graduation. His father was the Republican leader in Long Beach.

She will be questioned by kid gloved committee , and a back room deal will be made .Repukes get whatRead More they want and obama will fire her with a golden parachute ,end of story. This is America
Tideline May 23, 2013 at 12:04 pm
She also is still collecting a big paycheck for a job that she took the 5th on and would not answerRead More any questions. She should be immediately suspended without pay. She won't she is protected by the Obama umbrella. I predict she will be recalled, since she made a statement and that negated her 5th protections. When she refuses again next time, she should be found in contempt of Congress.

The victims are the poor defenseless cats and dogs in that shelter that Rescue Ink was using asRead More donation pawns, giving the cats and dogs scrimps while spending the large spoils on their own selfish selves. Why can't the North Shore Animal League take over the shelter and make it a second adoption site to complement their location in Port Washington?
andy napolitano May 23, 2013 at 05:51 pm
Well lets see now. Long Beach has always been known as such a corrupt city why shouldn't themRead More allowing these guys to illegally park their van anywhere they want come as such a surprise. Then to top it off let them open an animal shelter so they all can profit from it. I have lived in Long Beach for over 30 years & it has ALWAYS been known that city hall is not exactly on the up & up. So yeah I think we should be outraged at the City who allowed this to happen. And when this new scandel breaks wide open is everyone going to act so surprised? This is just the tip of the iceburg with this shelter & Rescue Ink.
Happy Daze May 23, 2013 at 04:49 pm
Wow such outrage. Did you really think these guys, who illegally parked their van in a handicappedRead More spot every night in the west end a few years back, were going to be the saviors they portrayed themselves to be? Oh and let's not blame this on the City because at least the City tried to provide an animal shelter for Long Beach.

I spoke with the construction mgr who told me that the entire bsmt, 1 st flr were flooded and 2 feetRead More of water onto the second floor. Foundation is sound, that is why they will be moving back in within 60 days. As bad as the damage looks, their financial picture seems even worse. Construction mgr did not seem to care about the owners situation, just finishing the work.
sun&sand May 23, 2013 at 07:05 am
The water flooded the first floor of apts. I am not sure if the entire foundation is sound--theRead More building was built on sand and several of the pilings might have to be reinforced. The 2nd floor and above was not flooded. The building sits on sand and is wide open to the elements. The people I know have been paying their maintenance charges which included RE taxes. However, the need for more money to fix everything is more than they have on hand.

Might actually be cheaper to buy a used RV. It doesn't need to really run or drive well. You canRead More find them on craigslist for as little as $1,000 dollars. Even if you change aout all matresses, clean up etc you could be all in for only the cost of a month or two rent. Then you can re-sell, donate to charity or give it away free when done and no problem if construction runs long you own it.

Just speculating that Schumer is buddies with Joe Panzarella & the shelter got finished whileRead More other places are still waiting. People can take what they want from that comment but it looks fishy.
AEA President May 23, 2013 at 09:40 am
what does this have to do with the shelter?
andy napolitano May 23, 2013 at 08:26 am
Yeah I've seen all the pictures of Joe "Panz" Panzarella with Chuck Schumer. Rescue InkRead More gets their shelter back but so many other things that are more important are still not up & running. The few animals that Rescue Ink had were in a safe warehouse so there was no real hurry to move them back to the building the shelter is in but they're back. "Politics make strange bedfellows" isn't that the saying? Chuck & Joe. Makes ya wanna go Hmmmmmmmm!!!!!!

What about the foot bridges to just get in the beach in the west end? And the people still out ofRead More their homes? And the phoney letters claiming outrageous percentages of damage (proven to be untrue) to get us to raise the houses for how many thousands to be reassessed higher taxes?

If you have an alarm system, you need to pay the city fee. They hired an outside company to manageRead More the payments so now the fee is expected to be collected.

I have heard 3 stories: the movie theatre owner had no flood insurance and will need to find $$ toRead More rebuild, no $$ so far; the theatre will be razed and will become home to 4 stores, the movie theatre will be renovated into a world class theatre with surround sound and ticket prices of $2 for children and $4 for adults except after 4PM.

some observations: one, applicable regulations for safety provide you cannot have mixed swimmingRead More and surfing beaches, so if the idea is to have both, you need to allocate. two, different beaches, because of bottom conditions, have very different waves -- ergo, the selection of beaches for surfing. three, on any given summer day with decent waves, and barely decent on a weekend, the number of surfers is dangerous. truly. I will not let my younger children surf/learn on those days. I won't even surf those days. four, on any given summer day, the number of people in the water, let alone actually swimming, is incredibly small in comparison to our total available beach real estate. five, as a general rule under this schedule, no swimmer should have to walk more than a few seconds to 2 minutes to be on a swimming beach. it is an allocation issue and not everyone is going to like the allocation, but that's often a part of the give and take of living in a community where resources are shared. folks who live near the train station have commuters parking in front of the their houses, sometimes with cars left for 2-3 days. similar for folks who live near the beach and can't get parking for themselves or at least their guests on a weekend. there are unbelievable issues with allocation of sports fields for practice and games. west-enders put up with bar traffic and a lot of other things. in any event, my two cents.
JPK May 20, 2013 at 01:46 pm
Couldnt agree more with your question/statement regarding the surf camps. They need to rotate also!Read More I think the camps are great for the kids and one day I hope to have a child in the camp, but they need to spread themselves around the beach!!!!
